• We are seeking a compassionate and skilled Nurse Practitioner to provide comprehensive primary care services in a collaborative clinical environment.
• This role focuses on preventive care, chronic disease management, and patient education.
• The provider will work closely with physicians and a multidisciplinary team to deliver high-quality, patient-centered care.

Job Responsibilities
• Perform patient assessments, diagnoses, and treatment planning
• Prescribe and manage medications
• Deliver preventive, behavioral, and community-based healthcare
• Educate patients on disease management and preventive practices
• Collaborate with multidisciplinary care teams
• Maintain accurate and compliant EMR documentation
• Participate in on-call rotation and provide cross-coverage as needed
• Engage in quality improvement initiatives, peer reviews, and team meetings
Qualifications
• Active California Nurse Practitioner license (BRN)
• Active DEA registration
• Minimum 2 years of clinical experience
• Background in Family Medicine, Internal Medicine, or Pediatrics preferred
• Strong clinical judgment and communication skills
• Ability to work effectively in a team-based environment
• Preferred: Bilingual (Spanish, Mandarin, Cantonese, Tagalog, Farsi, or Vietnamese)
Working Hours
• Outpatient clinic hours (typically Monday – Friday)
• Rotational after-hours on-call responsibilities
