Job description MUST BE LICENSED IN TX, FL, LA AND ABLE TO PRACTICE.
We are hoping for 16-25 hours a week at least (full time preferred)

The Nurse Practitioner (VSP) applies advanced education and clinical competencies to achieve optimal patient outcomes. The Nurse Practitioner (VSP) works on problems of diverse scope and complexity ranging from moderate to substantial.
This is a short-term role (6 MONTH) that will be focused on closing specific gaps for osteoporosis management in women.

Job Responsibilities

There may be opportunity to move to a similar role within CenterWell Medical group but that is not guaranteed. There also may be opportunity to cover other aspect of the program, such as covering PTO for APPs in CenterWell Medical group (phase 1).  There are no benefits tied to this position. The APP will spend their day performing the following duties:

Calling Humana members to discuss open care gaps and placing needed orders (specifically women who had a fracture in the last 6 months)

Creating and registering a patient record withing Athena

Verifying benefits

Gathering medical and surgical history

Verifying and entering medications

Coordinating management of members social needs to help facilitate gap closure – providing transportation services.

Connecting with outside partners to ensure testing is done prior to member specific timelines.

Ensuring adequate follow up- receipt of results, medication initiation, and PCP follow up.

Qualifications

Required Qualifications:
Bachelor's degree
 Masters in Science - NP
Active, unrestricted NP in two or more of the following states: NC, GA, KY, IL, LA, OH, VA, SC, TN, AZ, MO, NV prior to the start of employment
Excellent verbal and written communication skills
Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients
Fully engaged in the concept of “Integrated team-based care” model
Willingness and ability to learn/adapt to practice in a value-based care setting
Superior patient/customer service
Preferred Qualifications: 
Knowledge of HEDIS measures and quality indicators
Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment
