Job Responsibilities
Chronic Kidney Disease (CKD)
End-Stage Renal Disease (ESRD)
Hypertension
Electrolyte disorders
Nephrolithiasis and other renal conditions
Develop individualized treatment and disease-management plans
Conduct outpatient nephrology visits and dialysis rounding
Coordinate care between clinic, hospital, and dialysis settings
Educate patients and families on kidney disease, dialysis options, and transplant readiness
Collaborate with nephrologists, nurses, dietitians, and social workers
Manage dialysis-related complications and vascular access concerns
Maintain accurate clinical documentation and continuity of care
Qualifications
Active or eligible New Mexico NP license
National Board Certification (AANP, ANCC, or equivalent)
DEA license and prescriptive authority required
Strong clinical assessment and patient management skills
Nephrology experience preferred
