• We are seeking a dedicated Nephrology Nurse Practitioner to join a mission-driven healthcare organization focused on transforming kidney care.
• This role offers a dynamic practice environment across outpatient clinics, dialysis units, and hospital settings.
• The position emphasizes proactive care, early intervention, and coordinated treatment for patients with kidney disease.

Job Responsibilities
• Develop individualized care plans to slow disease progression and improve outcomes
• Optimize medication management and ensure continuity of care
• Conduct dialysis rounds (in-center and home modalities)
• Manage dialysis-related complications and vascular access issues
• Provide patient and family education on disease management, lifestyle changes, and treatment options
• Coordinate care with multidisciplinary teams including nephrologists, nurses, dietitians, and social workers
• Facilitate smooth transitions between clinic, hospital, and dialysis settings
Qualifications
• Active or eligible Nurse Practitioner license in New Mexico
• National Board Certification (AANP, ANCC, or equivalent)
• Valid DEA License with prescriptive authority
• Strong clinical background in nephrology or related field preferred
