Medical Group is seeking an experienced Nephrology Nurse Practitioner to join its collaborative team in the Hudson Valley. This role includes outpatient nephrology care in New Windsor, NY along with inpatient consultations and dialysis rounding at Hospital – Montefiore in Newburgh, NY. facility is a physician-owned, Patient-Centered Medical Home (PCMH) designated multispecialty group with over 35 locations, offering provider autonomy, flexible scheduling, and strong work-life balance.
Job Responsibilities
• Perform inpatient nephrology consultations and hospital rounding
• Conduct daily dialysis rounding for acute and chronic dialysis patients
• Order and interpret laboratory and imaging studies
• Collaborate with nephrologists, dialysis teams, and multidisciplinary staff
• Develop and implement comprehensive treatment plans
• Educate patients and families on kidney disease management and dietary modifications
• Maintain accurate EMR documentation
• Participate in quality improvement initiatives
Qualifications
• National certification (ANCC or AANP) – REQUIRED
• Master’s or Doctoral degree in Nursing – REQUIRED
• Valid DEA license – REQUIRED
• Minimum 2 years of nephrology experience – REQUIRED
• Experience with outpatient and inpatient nephrology care
• Knowledge of dialysis management and procedures
• Strong clinical and diagnostic skills
• Excellent communication and teamwork abilities
• BLS and ACLS certification – Preferred
