Nurse Practitioner – Post-Acute Care Opportunity
We are seeking a compassionate and clinically confident Nurse Practitioner to join a collaborative healthcare team providing high-quality, patient-centered care within post-acute care facilities.
This full-time opportunity offers a predictable Monday-Friday daytime schedule, strong clinical support, and the ability to make a meaningful impact caring for medically complex patients.
The ideal candidate enjoys building relationships with patients, families, nursing teams, and facility leadership while helping improve outcomes and reduce avoidable hospitalizations.
Responsibilities:
- Conduct daily patient rounds within assigned post-acute care facilities
- Perform comprehensive assessments and physical examinations
- Diagnose and manage acute and chronic medical conditions
- Develop and update individualized treatment plans
- Review laboratory results, imaging, and diagnostic testing
- Prescribe medications and treatments within scope of practice
- Collaborate with physicians, nursing teams, therapists, and care managers
- Participate in interdisciplinary care planning
- Maintain accurate electronic medical records
- Support quality improvement initiatives and patient outcome goals
Job Responsibilities

Promote preventative care and reduce hospital readmissions
Maintain regulatory and documentation compliance
Qualifications
Active Nurse Practitioner license
Current board certification as a Nurse Practitioner
Active DEA registration or ability to obtain
Strong clinical assessment and decision-making skills
Excellent communication and organizational abilities
Ability to work independently while collaborating with healthcare teams
Preferred Qualifications:
Experience in post-acute care

Working Hours
Consistent daytime schedule with no routine weekend shifts. Approximately six hours daily providing onsite patient care, with remaining time dedicated to documentation and care coordination.
