Job Responsibilities
• Manage acute and chronic conditions across diverse patient populations
• Conduct physical exams, preventive screenings, and follow-ups
• Develop individualized care plans and manage medications
• Collaborate with physicians and care teams for coordinated care
• Maintain accurate documentation using EMR systems
Qualifications
• Active California License (Required)
• DEA License (Preferred/Required depending on role)
• Experience in primary care or family medicine preferred
• Strong clinical and patient communication skills
