Nurse Practitioner – Geriatric / Long-Term Care

Full-Time | Weekday Schedule | Meaningful, Relationship-Based Care

We are seeking a dedicated Nurse Practitioner (FNP or AGNP) to provide patient-centered primary care services within long-term care and skilled nursing settings. What You’ll Do

  • Provide comprehensive assessments, diagnoses, and ongoing management of residents in long-term care/skilled nursing facilities

  • Collaborate with facility staff, physicians, and interdisciplinary teams

  • Develop individualized care plans to support optimal patient outcomes

  • Maintain accurate and timely documentation

  • Deliver care with compassion, patience, and professionalism

What We’re Looking For

  • Active APRN-NP license (Required)

  • Experience in geriatrics, long-term care, or primary care preferred, but new graduates are encouraged to apply

  • Strong commitment to delivering high-quality, patient-focused care

  • Excellent communication, critical thinking, and clinical judgment
