A dynamic community-based health center in Manhattan is seeking a dedicated Nurse Practitioner to join its multidisciplinary team. This full-time role focuses on providing compassionate, comprehensive care to adult patients, including those living with HIV, in an outpatient setting. The center is a Federally Qualified Health Center (FQHC) and a recognized Patient-Centered Medical Home, offering a wide range of bilingual health services.
Key Responsibilities:
- Deliver primary care services with a focus on HIV treatment and prevention in an ambulatory care environment.
- Participate in the center’s on-call rotation, providing telephone coverage as scheduled.
- Support quality improvement initiatives and contribute to clinical program development.
- Collaborate with other healthcare professionals to ensure continuity of care and optimal patient outcomes.
- Engage in staff education and development activities to promote best practices.
- Maintain accurate and timely documentation in the electronic health record system.
Requirements:
- Active New York State license as a Nurse Practitioner.
- Must hold an active NYS Medicaid/Medicare provider number.
- Strong communication and interpersonal skills; bilingual Spanish is a plus.
- Minimum of 2–3 years of clinical experience in adult medicine preferred.
- Willingness to work toward HIV certification if not already certified.
