Acute Care Opportunity in Independence, MO
Job at a glance
Job description
We are recruiting an acute care nurse practitioner for an employed, outpatient primary care position with a value-based senior care group in Independence, Missouri. Full-time, onsite.
The role
This is the acute-care arm of a center care team rather than a paneled primary care position. You would carry the urgent side of the practice so paneled providers can hold their schedules.
- Walk-in visits, same-day sick visits, and hospital follow-up
- Communicating plans of care back to members' primary care providers
- Working with center leadership to identify members who need acute support
- Directing and partnering with medical assistants and the wider care team
- Supporting patient access and keeping wait times down
- Participating in clinical and quality-review meetings, and in community outreach
Practice setting
- Outpatient primary care and consulting under physician supervision and collaboration agreements
- Purpose-built neighborhood centers serving a Medicare-age population
- Role covers several centers a short distance apart within the same market
Important to know
- This position carries night, weekend, and holiday call on a schedule set by the employer
What we're looking for
- Active, non-probationary Missouri professional nursing license
- Minimum one year of primary care experience in internal medicine, geriatrics, or family medicine
- Current BLS and ACLS certification
- Comfort managing a predominantly Medicare-age, medically underserved panel with multiple chronic conditions
- Strong command of examination technique and diagnostics
- Willingness to travel between nearby centers in the market
