Hospitalist Nurse Practitioner or Physician Assistant - Acute Care at Home Work Model
Are you an NP or PA-C with a Hospitalist skill set seeking a full-time, ongoing contract in the Walnut Creek, CA area? GHR Healthcare is partnered with a company seeking a Provider to support them doing in-home patient visits.
Contract Duration: Contract will be renewed annually
Schedule: 10-hr shifts, 7on 7off
Territory: Walnut Creek, CA
Compensation: $110/hr + mileage reimbursement

Job Responsibilities

The NP or PA serves as the point of contact representing the program to the patients and
family in person, collaborating with the attending physician in the organizations Medical Command
Mission Control and the patient’s primary care physician (PCP).
The NP or PA performs the initial face-to-face evaluation and assessment of potential patient admissions, and
upon admission, the NP or PA will collaborate with the attending physician and the PCP to develop and
implement an acute plan of care in the home.
The NP or PA will possess expert, complex, clinical decision-making skills; triage and collaboration skill and will
display a high-level of comfort with providing care in a non-traditional setting centered on the following
key responsibilities:
• Facilitating the triaging of eligible, appropriate patients referred for a potential admission
facilitated by company guidelines; obtaining additional diagnostic and specialty
consultation when needed
• Providing inpatient level acute care in the home setting in collaboration with the home hospital
attending physician and RN
• Completing the patient referral and providing safe handoffs to the attending physician and RN
• Communicating with the patient and family concerning the company model, incorporating
patient and family values and goals in the plan of care
• Navigating patients and families across the continuum of the company phases of care.
• Navigating barriers for patients and those within the system to accomplish optimal patient
experience and outcomes
• Promoting a collaborative relationship and effort between the attending physicians, RN team,
and supplier providers
• Serving as a role model for professional nursing practice
Qualifications
At least 1 year of hospitalist experience
