Locum Tenens opportunity for an experienced Nurse Practitioner to provide home-based hospice and palliative care to patients with serious or life-limiting illnesses in the Escondido, California area.
The ideal candidate is compassionate, independent, and comfortable providing care in the home setting, conducting goals-of-care conversations, managing symptoms, and collaborating with an interdisciplinary hospice team.

Job Responsibilities
Perform comprehensive patient assessments
Develop and manage individualized care plans
Assess and manage symptoms associated with serious and life-limiting illnesses
Conduct goals-of-care discussions and advance care planning
Assist with POLST and advance directive discussions
Educate patients, families, and caregivers
Coordinate care with the interdisciplinary hospice team and community providers
Serve as a clinical resource for home health aides and the care team
Participate in interdisciplinary team meetings
Maintain accurate documentation in the EMR
Collaborate with the Hospice and Palliative Care Medical Director
Participate in rotating weekend and on-call coverage
Qualifications
California RN license required
National NP certification through AANP or ANCC
MSN required
DEA registration required
NPI required
BLS certification required
3+ years of hospice or palliative care experience preferred
ACHPN certification preferred at hire and required within one year
Valid California driver's license and appropriate auto insurance required
Spanish bilingual skills are a plus
Working Hours
8:00 AM–5:00 PM
Rotating weekend coverage
Approximately one Sunday per month with a weekday off
Occasional on-call coverage
Participation in patient coverage as needed
