• Provide home-based hospice and palliative care to patients with serious, life-limiting, or terminal illnesses.
• Conduct comprehensive face-to-face patient assessments and develop individualized care plans.
• Manage symptoms and support patients and families throughout the hospice care journey.
• Participate in goals-of-care discussions, advance care planning, and patient/family education.
• Collaborate with the interdisciplinary hospice team, Medical Director, community physicians, and other care providers.
• Serve as a clinical resource for home health aides and the hospice care team.
• Complete approximately 45–50 patient visits per month.
• Document all patient encounters and clinical activities in the My Unity EMR.

Job Responsibilities
• Conduct patient assessments and evaluate changes in clinical condition.
• Develop, implement, and update individualized hospice and palliative care plans.
• Assess and manage pain, symptoms, and other end-of-life care needs.
• Conduct goals-of-care conversations and provide advance care planning support.
• Assist with POLST and advance directive discussions and documentation.
• Educate patients, families, caregivers, and members of the care team.
• Coordinate care with the interdisciplinary team and community healthcare providers.
• Participate in interdisciplinary team (IDT) meetings and quality initiatives.
• Provide clinical guidance and support to home health aides and hospice staff.
• Communicate effectively with the Hospice & Palliative Care Medical Director.
• Complete accurate and timely documentation in the My Unity EMR.
• Participate in rotating weekend coverage and occasional on-call responsibilities as required.
• Support 24/7 patient coverage when needed.
Qualifications
• Current California Registered Nurse (RN) license.
• DEA registration.
• NPI.
• MSN or appropriate advanced nursing degree.
• AANP or ANCC certification.
• Current BLS certification.
• Minimum 3 years of hospice or palliative care experience.
• ACHPN certification required within one year; preferred at hire.
• Valid California driver's license and reliable transportation.
• Current automobile insurance.
• Spanish bilingual skills are a plus.
• Must be compassionate, independent, team-oriented, and comfortable discussing end-of-life care.
Working Hours
• Standard schedule: 8:00 AM–5:00 PM.
• Once a month, work a Sunday and take a weekday off in exchange.
• Rotating weekend coverage is required.
• Occasional on-call coverage may be required.
• Participation in 24/7 patient coverage when needed.
• Start Date: September 28, 2026.
• Contract Duration: 13 weeks.
