Annual Salary: $100k-$125k (varies depending on years of experience)

A well-established nonprofit hospice organization in the Hudson Valley region is seeking a compassionate and skilled Nurse Practitioner to join its interdisciplinary care team. This role involves providing high-quality, patient-centered care to individuals facing life-limiting illnesses in a variety of settings, including private residences, skilled nursing facilities, and a dedicated hospice residence.

You’ll be part of a collaborative clinical team that values your expertise and input, working alongside physicians, nurses, social workers, and other professionals to ensure comfort and dignity for patients and their families.

Key Responsibilities:

  • Perform home visits to assess symptoms, manage pain and other distressing conditions, and evaluate hospice eligibility.
  • Collaborate with intake and medical staff to review clinical documentation and determine appropriate diagnoses and care plans.
  • Provide guidance and support to nursing staff, acting as a clinical resource for complex cases and symptom management.
  • Participate in outreach efforts and educational initiatives within the community to promote awareness of hospice services.
  • Serve as an on-call provider, including medication management (controlled substances included), clinical decision-making, and completion of necessary documentation such as death certificates.
  • Engage in goals-of-care discussions with patients and families, ensuring alignment with their values and preferences.

Requirements:

  • Master’s or Doctorate degree in Nursing with Nurse Practitioner certification.
  • Active NP licenses in New York State.
  • Valid NY driver’s license and current auto insurance.
  • Minimum of two years’ experience as a Nurse Practitioner, ideally in home-based care or primary care settings.
  • Hospice or palliative care experience strongly preferred; certification in hospice care is a plus.
