Now Hiring: Nurse Practitioner/Physician Assistant – Home-Based Primary Care (Fargo, ND Region)

We are a non-profit, community-owned organization with 40+ years of experience in delivering compassionate, patient-centered care across North Dakota and Minnesota. Our team is supported by full-time, dedicated physicians and nurse practitioners who specialize in hospice and palliative medicine.
Location: Fargo, ND
Schedule: Flexible scheduling with autonomy to tailor visits to patient needs (M-F)
Team Meetings: On-site once weekly
Day-to-Day Responsibilities:

50/50 split between home visits and assisted living facility visits

Expectation to see 4–8 patients per day depending on acuity and travel time

Collaborate with interdisciplinary care teams

Deliver exceptional, mission-aligned care to patients in a variety of home-based settings

Job Responsibilities
50/50 split between home visits and assisted living facility visits

Qualifications
Minimum of 3 years of NP/PA experience in primary care with a focus on geriatrics
Strong preference for prior home-based care experience
Current licensure in North Dakota and Minnesota (or ability to obtain)
Active DEA license
Valid driver’s license and auto insurance
Ability to travel regularly within the designated territory
Experience in hospice and/or palliative care is highly preferred
