Location: A city located in northwest Ohio, known for its rich history, economic growth, and vibrant community. It's a part of the larger Toledo metropolitan area.
Facility: A 150-bed, not-for-profit acute care facility providing comprehensive healthcare services.

Opportunity:
- Seeking a compassionate and experienced Nurse Practitioner with a strong commitment to palliative care, hospice, and geriatrics. Join a community-based nonprofit that truly values its providers and their impact on patient lives.
- Deliver care in a 75% inpatient / 25% outpatient setting
- Provide palliative care services primarily at hospital
- Work Monday–Friday, 8:00 AM–5:00 PM, managing approximately 6 patients per day
- Practice autonomously to manage symptoms and chronic diseases
- Lead goals of care discussions and provide advanced care planning
- Round on inpatient hospice patients as part of daily duties
- Collaborate with hospitalists, who manage patient admissions
- Function within an open ICU environment
- Participate in a rotating on-call schedule
- Role requires experience as an advanced provider in acute care, palliative care, or hospice
- Ideal candidates will have a strong passion for palliative care, hospice, and geriatrics
- Competitive base salary based on experience and training (to be discussed at interview)
