Annual Salary: $130k – $185k (varies depending on years of experience)
A respected healthcare organization specializing in palliative and end-of-life care is seeking a compassionate and experienced Nurse Practitioner to join their dedicated hospice team. This role is ideal for someone who values collaboration, empathy, and the opportunity to make a meaningful difference in patients’ lives during their most vulnerable moments.
The Nurse Practitioner will provide direct patient care within a multidisciplinary, team-based environment, working closely with physicians, nurses, social workers, and spiritual care providers to ensure comfort, dignity, and quality of life for every patient.
Key Responsibilities:
- Deliver high-quality, patient-centered hospice and palliative care to individuals in inpatient, home, or community-based settings.
- Assess, diagnose, and manage symptoms related to pain, chronic illness, and end-of-life care in collaboration with attending physicians.
- Participate in the development and implementation of individualized care plans that honor patient goals, values, and comfort.
- Maintain thorough and timely clinical documentation in compliance with organizational and state standards.
- Provide emotional support and education to patients, caregivers, and families about disease progression, medication management, and comfort measures.
- Serve as a clinical resource to nursing and support staff, promoting best practices in symptom control and compassionate communication.
- Coordinate with interdisciplinary teams to ensure continuity of care across settings.
- Conduct home visits or facility rounds as needed, supporting patients wherever they receive care.
- Participate in quality improvement and staff development initiatives related to hospice and palliative medicine.
- Uphold all standards of care, ethics, and professionalism consistent with state regulations and nursing practice guidelines.
Qualifications:
- Active New York State RN and NP licenses (required)
- Minimum 2 years of clinical nursing experience
- At least 1 year of hospice or home health experience strongly preferred
- Excellent communication and interpersonal skills with the ability to work collaboratively in a team-driven environment
- Valid New York State driver’s license preferred (for field visits)
- Demonstrated empathy, emotional intelligence, and commitment to patient-centered care
