Nurse Practitioner (NP) – In-Home Risk Adjustment Assessments
Location: Lane County, OR

What You’ll Do

Conduct in-home annual wellness visits and risk adjustment assessments

Evaluate patients with chronic conditions through:

Comprehensive physical exams

Medication reviews
-
Cognitive, mental health, and fall risk screenings

Social and functional assessments
-

Educate patients on health maintenance topics such as medication adherence, pain management, and chronic disease self-care

Help close quality care gaps (labs, screenings, immunizations)

Accurately document assessments in a user-friendly EMR
Qualifications
-
Active, unrestricted Nurse Practitioner license in OR

3+ years of experience in patient care (geriatrics, primary care, or home health preferred)

Previous experience in risk adjustment or in-home assessments preferred

Comfortable working independently in a field-based environment

Must have reliable transportation and a willingness to travel
