A well-established healthcare organization is seeking a compassionate Palliative Care NP to join their home care team. Based in Brooklyn and Manhattan, this role involves providing care to patients in their homes and providing palliative care consults at a health and rehabilitation center in Manhattan.
Responsibilities:
- Providing palliative care consultation services and NP-appropriate services to patients enrolled in hospice.
- Engaging in medical history-taking, physical examinations, formulating care plans and treatment recommendations.
- Providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and use of organizational materials and policies.
Qualifications:
- Active ANP, FNP, or GNP license in the state of New York
- Experience with Palliative Care, Home Care, and/or Geriatrics
Desired Skills:
- Managed Care experience
- Hospice and Palliative Care Certification
$115,000-143,000 base salary + 15k sign on or student loan assistance + comprehensive benefits
