Nurse Practitioner, Infusion- Bellflower

The Infusion Nurse Practitioner provides specialized IV therapy treatment to patients in ambulatory settings such as clinics, home care, and ambulatory infusion centers. This role prepares, verifies, and administers chemotherapy, immunotherapy, and supportive oncology and infusion medications in accordance with provider orders and institutional protocols. This role maintains clinical practice standards and supports staff development. This role is integrated into services and operations with nursing standards of practice, service priorities, and performance/outcome measures, as well as with business, departmental, and organizational goals and objectives.

Minimum Requirements

• Graduation from an accredited Baccalaureate school of nursing is required.

• Current and active licensure in the State of California is required.

• National board certification (FNP, AGNP, Acute Care NP, or specialty-relevant) is required.

• Training in the diagnosis and management of complex medical conditions, completed.

• Current DEA registration is required.

• ONS/ONCC Chemotherapy & Immunotherapy Certification is required.

• Maintaining competency or certification in oncology/chemotherapy administration (ONS/ONCC or equivalent).

• A minimum requirement of a valid ACLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

• Experience in infusion therapy, Emergency, ICU, oncology, or critical care settings preferred.

• Previous experience with computer-based systems is required; Electronic Medical Records experience is preferred.

• Bilingual in Spanish and English is preferred.

Skills and Abilities

• Oral communication skills are necessary to give clear, concise instructions to patients and/or families regarding treatment, and to consult with medical staff

• Ability to teach and present recommendations on programs, and to interact with department staff, other departments, patients, and families with good public relations skills, and to understand oral instructions.

• Reading and writing skills in the English language are necessary to evaluate patient charts and documents, participate in research programs, collect data, and understand written instructions.

• Courteous, responsive, and overtly hospitable to customers and employees.

• Ability to work shifts that include days, evenings, and weekends, depending on the setting.

Physical Requirements

• Ability to sit, stand, stoop, reach, bend, and lift up to 10 pounds as necessary to carry out routine duties.

• Hand and wrist dexterity to utilize a computer.

• May be required to stand or sit for extended periods at the computer.

• High energy is required to handle multiple tasks and projects.

Environmental Conditions Critical to Performance

• Work is in a medical office or home environment.

• Low risk for contamination when adhering to Universal precautions related to the performance of treatments. However, exposure to medications, blood, and bodily fluids may occur.

• Travel expectations if home infusion is part of the role. Shifts (days, evenings, weekends depending on setting).

Essential Job Functions

• Performs and documents health appraisals on clinic patients, including history, physical assessment, and developmental evaluation as indicated, also orders laboratory work and/or x-rays as indicated for patient assessment, and identifies findings that require consultation and referral.

• Initiates preventative health counseling and health promotion measures, including immunizations.

• Assesses problems and/or needs when patients telephone between clinic visits, arranges for appropriate follow-up on referral, consults with the physician when indicated, and re-orders patients' medication according to physician orders.

• Manages the delivery of nursing services and outpatient care, which is integrated with business plans and meets or exceeds cost, quality, clinical utilization standards, and performance measures.

• Conducts pre-treatment assessments, including labs, vital signs, symptom review, and toxicity screening.

• Verifies treatment parameters (ANC, renal/hepatic function, weight-based dosing, etc.) before administration.

• Administers IV infusions (e.g., biologics, antibiotics, hydration, chemotherapy, depending on scope). Including management of peripheral IVs; care for central lines (PICC, ports).

• Monitors patients continuously during and after infusions for chemotherapy-related reactions or complications. Recognizes and intervenes promptly in hypersensitivity or anaphylactic reactions following established emergency protocol

• Follows all hazardous drug safety standards (e.g., USP <800>, PPE requirements, spill management).

• Ensures proper handling, priming, administration, and disposal of chemotherapy and hazardous materials.

• Performs independent double-checks for all chemotherapy agents, including dosage, route, sequencing, and patient identification.

• Documents, therapy administration, patient tolerance, side effects, and supportive care provided. Record symptoms, assessments, and any escalations or interventions in the EMR following oncology documentation standards

• Educates patients and families on treatment plans and medication side effects.

• Ensures staff provide the highest quality of care and are following the Nurse Practice Act, NCQA, TJC, ACHC, federal, state, and local requirements.

• Collaborates with medical leadership, physicians, and pharmacists in establishing, implementing, maintaining, and improving patient care. Communicates changes in patient status, emergent symptoms, or treatment barriers promptly.

• Acts as a patient advocate, resolving patient care issues.

• Supports service integration programs with Dental, PACE/Case Management Programs, Youth Services, and HIV Services.

• Implements and ensures compliance with policies and procedures that support the organization’s goals and business objectives.

• Review the clinic patient laboratory work and notify the clinic nurses of abnormal findings or consult directly with Physicians.

• Acts as liaison with outside agencies, physicians, and schools, referring patients to outside resources as indicated.

• Participates in the Quality Management Program, Utilization Review, Peer Review, AltaMed Medical Provider activities, and AltaMed's On-call schedule.

• Assists any internal or external consumers with any problems they may have.

• Participates in the AltaMed on-call schedule, which would encompass all hours of work, including days, nights, weekends, and holidays.

• Performs other duties as assigned.
