About the Opportunity
A well-established medical group on Long Island is seeking an experienced Nurse Practitioner or Physician Assistant for an evening shift position at a skilled nursing facility in Bay Shore, NY. This W-2 role suits an APP who is confident in long-term care medicine, wants a consistent PM schedule, and enjoys working closely with facility staff to deliver high-quality patient care.
Position Highlights
- Shift: PM / Evening, 4:00 PM – 10:00 or 11:00 PM
- Compensation: $160,000 – $170,000 annually
- Employment Type: W-2
- Setting: Skilled Nursing Facility (SNF)
- Location: Bay Shore, NY (Suffolk County, Long Island)
- Patient Volume: About 25 patients per shift
Responsibilities
- Manage a census of about 25 SNF patients per shift
- Complete progress notes and documentation
- Evaluate and treat acute changes in condition during evening hours
- Coordinate care with nursing, rehab, and facility staff
- Collaborate with the supervising physician on care plans
Requirements
- Prior SNF / long-term care experience is required
- Active NY NP or PA license in good standing
- Active DEA registration
- Comfortable managing a high-volume census independently during evening hours
