- Bilingual and or ESRD experience a huge PLUS
About the Role: You'll work within a patient-centered, value-based care model focused primarily on adults 55+, including high-risk and medically complex patient populations. Responsibilities include:
- Conducting comprehensive health assessments and physical exams
- Diagnosing and managing acute and chronic conditions
- Delivering preventive care and patient education
- Identifying care gaps and opportunities to improve patient outcomes
- Providing comprehensive, coordinated care for geriatric patients
