Job description
We are seeking a Nurse Practitioner (NP) to manage the ongoing medical needs of residents in skilled nursing and long term care facilities throughout the Eugene area.
The NP will evaluate and manage acute and chronic medical conditions, provide ongoing clinical care, and collaborate with physicians and interdisciplinary facility teams to deliver individualized, high quality care.
Job Responsibilities
• Evaluate, diagnose & manage acute and chronic medical conditions
• Develop individualized treatment plans & prescribe medications and therapies within scope of practice
• Order, review & interpret laboratory tests, imaging & other diagnostic studies
• Address diagnostic results promptly
• Provide post hospital follow up & transitional care
• Assess changes in residents’ cognitive, behavioral, functional & medical status
• Educate residents, families & caregivers regarding diagnoses, treatment options & care plans
• Participate in interdisciplinary care planning with nursing, therapy, dietary, social services & administrative teams
• Coordinate care with collaborating physicians & specialists
• Support efforts to reduce avoidable ED visits & hospital readmissions
• Maintain accurate & timely EMR documentation
• Comply with CMS, state & facility requirements
Qualifications
• Active, unrestricted Oregon NP license or ability to obtain
• National board certification or board eligibility by start date
• DEA registration & prescriptive authority, or ability to obtain
• National Provider Identifier (NPI)
• Eligibility to participate in Medicare & Medicaid
• Current BLS certification
Skills
• Geriatric, Internal Medicine, Family Medicine or post acute care experience
• Experience managing medically complex patients & chronic conditions
• Familiarity with CMS documentation & regulatory requirements
