Job Responsibilities
Provide compassionate, relationship-based care to older adults in an assisted living environment
Build strong relationships with patients, families, and facility staff through communication and consistent presence
Manage chronic conditions, evaluate acute concerns, and create individualized treatment plans
Collaborate with facility care teams to provide coordinated, comprehensive support
Apply geriatric, home health, or hospice experience to improve outcomes for medically complex patients
Maintain flexible availability for patient visits and on-site care needs
Complete accurate clinical documentation while maintaining high standards of quality and safety

Qualifications
Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in Georgia
Master’s degree from an accredited NP or PA program
Minimum of 3 years of clinical experience
Strong preference for experience in geriatric medicine, hospice, home health, or complex care management
Active DEA license or eligibility to obtain
Ability to work on-site in Loganville and surrounding areas
