Job Responsibilities
Conduct comprehensive assessments, histories, and physical examinations
Diagnose and manage acute and chronic medical conditions
Develop and update individualized treatment plans
Monitor changes in patient condition and coordinate timely interventions
Order and interpret laboratory testing, imaging, and diagnostic results
Prescribe medications and treatments within scope of practice
Collaborate with physicians, nursing teams, therapists, case managers, and facility leadership
Participate in interdisciplinary care planning and quality improvement initiatives
Maintain accurate documentation within electronic medical records
Promote continuity of care and reduce unnecessary hospital readmissions
Qualifications
Active Nurse Practitioner license
Board certification as a Nurse Practitioner
Active DEA registration or ability to obtain
Strong clinical assessment and diagnostic skills
Excellent communication and interpersonal abilities
Ability to work independently while collaborating with a healthcare team
Preferred Qualifications:
1+ year of Nurse Practitioner experience preferred
Experience in any of the following:
Post-acute care
Skilled nursing facilities
Rehabilitation
Geriatrics
Internal medicine
Primary care
Hospital medicine
Emergency medicine
Critical care
New graduate Nurse Practitioners with strong clinical backgrounds are encouraged to apply.
Working Hours
Monday–Friday
Daytime schedule
No routine weekend shifts
Approximately 6 hours/day providing onsite patient care
Remaining time dedicated to documentation and care coordination
