We are seeking a compassionate, clinically confident Nurse Practitioner (NP) to join a collaborative healthcare team providing high-quality, patient-centered care in post-acute care facilities.

Job Responsibilities
Conduct daily patient rounds within assigned post-acute care facilities.
Perform comprehensive patient assessments, medical histories, and physical examinations.
Diagnose, treat, and manage acute and chronic medical conditions.
Develop, implement, and update individualized treatment plans.
Monitor changes in patient condition and coordinate timely interventions.
Order, review, and interpret laboratory tests, imaging studies, and other diagnostic results.
Prescribe medications and treatments in accordance with state regulations and scope of practice.
Collaborate closely with physicians, nursing staff, Directors of Nursing, therapists, case managers, and facility leadership.
Participate in interdisciplinary care planning, case conferences, and quality improvement initiatives.
Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR).
Promote continuity of care through proactive patient management and effective communication with the care team.
Support initiatives that improve clinical outcomes, patient satisfaction, and reduce unnecessary hospitalizations.
Qualifications
Active Nurse Practitioner license in the applicable state.
Current board certification as a Nurse Practitioner.
Active DEA registration or eligibility to obtain one.
Strong clinical assessment, diagnostic, and decision-making skills.
Excellent communication, organizational, and interpersonal skills.
Ability to work independently while collaborating effectively within an interdisciplinary healthcare team.
Preferred Qualifications:
One or more years of Nurse Practitioner experience preferred.
New graduate Nurse Practitioners are encouraged to apply.

Schedule:
Full-time, Monday–Friday
Consistent daytime schedule with no routine weekend shifts
Approximately six hours each day providing on-site patient care within assigned facilities
Remaining time dedicated to documentation, care coordination, and other administrative responsibilities
