We are seeking an experienced Nurse Practitioner or Physician Assistant who enjoys caring for older adults and building trusted relationships with patients, families, and care teams.
This role provides medical care within assisted living communities, focusing on comprehensive senior care, chronic disease management, preventive care, and coordination with facility partners.
The ideal provider is compassionate, dependable, comfortable working independently, and passionate about improving healthcare experiences for seniors.
Job Responsibilities
Build strong relationships with patients, families, and facility staff
Manage chronic conditions and evaluate acute medical concerns
Develop individualized treatment plans based on patient needs
Perform patient assessments and follow-up visits
Coordinate care with facility teams and healthcare partners
Support medically complex patients through proactive care management
Maintain accurate clinical documentation
Deliver safe, high-quality, patient-centered care
Qualifications
Current, unrestricted Nurse Practitioner or Physician Assistant license in Georgia
Master's degree from an accredited NP or PA program
Minimum of 3 years of clinical experience
Active DEA license or eligibility to obtain
Ability to provide onsite care in Loganville and surrounding communities
