Job description Client is seeking a compassionate and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide relationship-centered care for older adults in premier assisted living communities throughout the Loganville, Grayson, and Sugar Hill areas.

Job Responsibilities

Key Responsibilities:
Provide compassionate, relationship-based medical care to older adults in assisted living communities
Build trusted relationships with patients, families, and facility staff through consistent communication and presence
Manage chronic conditions, evaluate acute concerns, and develop individualized treatment plans
Coordinate care with facility teams and healthcare partners to ensure seamless patient support
Apply experience in home health, hospice, or geriatric medicine to improve outcomes for medically complex patients
Conduct patient visits and maintain an appropriate on-site presence based on patient needs
Document clinical encounters accurately and maintain high standards of quality and safety

Qualifications

Qualifications:
Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in Georgia
Master’s degree from an accredited NP or PA program
Minimum of 3 years of clinical experience
Strong preference for experience in geriatric medicine, hospice, home health, or complex care management
Active DEA license or eligibility to obtain
Ability to work on-site in Loganville and surrounding communities
