We’re seeking an experienced NP to join a well-established PACE program in Fayetteville, North Carolina, providing comprehensive, patient-centered care to the geriatric population within a collaborative interdisciplinary environment.

What You’ll Be Doing

✅ Provide primary medical care in collaboration with PACE physicians
✅ Perform initial, annual, semi-annual, and as-needed participant assessments
✅ Develop individualized care plans within the interdisciplinary team model
✅ Evaluate acute illnesses and manage chronic conditions
✅ Deliver preventive care, screenings, immunizations, and monitoring
✅ Prescribe medications and therapies appropriately
✅ Coordinate specialist referrals and transitions of care
✅ Participate in discharge planning
✅ Educate patients, families, and caregivers
✅ Contribute to staff education and clinical policy development
✅ Maintain accurate and compliant clinical documentation
✅ Participate in on-call rotation responsibilities

Candidate Requirements

✅ Graduate of an accredited PA program
✅ Active North Carolina PA License or willing to obtain
✅ Minimum 2 years PA experience required
✅ Geriatric experience preferred
✅ Experience with frail/elderly patients or willingness to complete PACE training
✅ BLS Certification required
✅ DEA Registration required
✅ Strong clinical and independent decision-making skills
✅ NCCPA Certification required
✅ Clean malpractice/license history highly preferred
✅ NPDB Self Query Report within 30 days required at presentation
