Physician's Services Group (PSG) is seeking a compassionate and motivated Nurse Practitioner (NP) to join our growing Post-Acute Care team in the North Augusta, South Carolina area. This full-time opportunity offers a stable weekday schedule, clinical autonomy, and the ability to build meaningful relationships with patients while improving outcomes and reducing unnecessary hospitalizations.
The provider will deliver in-person care within Skilled Nursing and Post-Acute Care facilities while collaborating closely with physicians, nursing staff, and interdisciplinary care teams.
Job Responsibilities
Conduct patient assessments, follow-up visits, and chronic disease management
Collaborate with physicians, nursing staff, and facility leadership
Maintain accurate and timely EMR documentation
Assist in reducing hospital readmissions through proactive patient management
Develop individualized treatment plans focused on preventative care and quality outcomes
Qualifications
Board Certified (ANCC, AANP, or equivalent)
Strong clinical judgment and communication skills
Experience in post-acute, hospital, primary care, or outpatient settings preferred
Passion for geriatric medicine and chronic disease management
