• We are seeking a compassionate Nurse Practitioner to provide high-quality, patient-centered care to residents in an assisted living setting.

Job Responsibilities
• Develop personalized care plans with interdisciplinary teams
• Manage acute and chronic conditions in geriatric patients
• Order and interpret labs and diagnostic tests
• Prescribe medications within scope of practice
• Educate residents and families on health maintenance and disease prevention
• Maintain accurate documentation in eClinicalWorks EMR
• Participate in quality improvement initiatives
Qualifications
• Active New York State NP license and DEA registration required
• Must be a participating Medicare/Medicaid provider
• Minimum 3+ years of clinical experience (geriatrics or long-term care preferred)
• Strong patient care skills with a compassionate approach
Working Hours
• Preferably 2 days per week
• No nights, no weekends
• On-site only
Skills
• Strong clinical assessment and diagnostic abilities
• Excellent communication with patients and families
• Ability to work in interdisciplinary care teams
• Proficiency in EMR systems (eClinicalWorks preferred)
