Job Title: Nurse Practitioner – Wound Care & Primary Care (Long-Term Care)
Location: Multiple sites across Ohio (Piqua, Toledo, Greenville, Celina, Cridersville, Spaulding, Defiance, Ottawa)
Schedule: Monday–Friday | 8:00 AM – 5:00 PM

Position Overview

We are seeking a Nurse Practitioner to provide wound care and primary medical services to residents in skilled nursing and long-term care facilities. This role focuses on delivering high-quality, patient-centered care while collaborating with interdisciplinary teams and facility staff to manage complex and chronic conditions.

Key Responsibilities

  • Deliver wound care and primary care services to long-term care residents

  • Assess, diagnose, and treat acute and chronic conditions

  • Prescribe medications, including controlled substances, in accordance with Ohio law

  • Order, review, and follow up on diagnostic tests and treatments

  • Stage pressure injuries according to NPIAP guidelines

  • Manage complex wounds (diabetic, arterial, venous, mixed etiology, Kennedy ulcers)

  • Utilize advanced wound care products (collagens, alginates, foams, silver dressings, etc.)

  • Use wound vacs and pressure-relief equipment as appropriate

  • Communicate regularly with nursing staff regarding patient status and care plans

  • Collaborate closely with primary care physicians and interdisciplinary teams

  • Document visits and care plans accurately within EMR systems

Required Qualifications

  • Active, unrestricted Nurse Practitioner license in Ohio

  • Board Certification

  • Clinical background in adult, family, or geriatric care

  • Experience treating chronic or complex wounds

  • Ability to manage patients with multiple comorbidities

  • Experience in long-term care settings

  • Proficiency with EMR systems

  • Strong collaboration and communication skills

Preferred:

  • Wound care certification (or willingness to obtain)

  • Basic medical coding knowledge

  • DEA license

Compensation & Benefits

  • Salary: $115,000 – $130,000
