Job description
Join a mission-driven PACE (Program of All-Inclusive Care for the Elderly) team providing comprehensive, community-based healthcare to frail and elderly adults. This rewarding Nurse Practitioner role focuses on helping seniors maintain independence, dignity, and quality of life while delivering full-spectrum geriatric primary care.
Work within an interdisciplinary team coordinating medical and supportive services across the continuum of care β including clinic visits, home visits, and transitional care planning. Ideal for providers passionate about geriatric medicine, chronic disease management, and community health.
Job Responsibilities
β’ Develop and implement individualized care plans
β’ Evaluate and treat acute and chronic conditions
β’ Conduct home visits as clinically indicated
β’ Participate in discharge planning and care transitions
β’ Coordinate services across the continuum of PACE care
β’ Collaborate with interdisciplinary team members
β’ Assist in development and review of clinical policies
β’ Participate in provider on-call rotation
β’ Ensure compliance with regulatory and quality standards
Qualifications
β’ Active Massachusetts NP License β Required
β’ DEA Registration β Required
β’ CDS Certification β Required
β’ BLS Certification β Required
β’ Recent NPDB Self-Query Report β Required
β’ Experience working with frail or elderly populations β Required
β’ Ability to travel within service area β Required
