A well-established healthcare organization is seeking a compassionate Field Nurse Practitioner to join their home care team. Covering the Manhattan and Bronx territories, this role involves providing care to patients in their homes.
Responsibilities:
- Provide comprehensive assessment, including multifactorial complex physical examination and psychosocial needs, self-management education & tools to better manage patient's multiple conditions
- Provide and coordinate primary care services to an assigned the at-risk panel of geriatric patients who reside in the community
- Ensure development of care planning and access to preventive care services in accordance with member and plan health outcome goals and objectives
- Act as a clinical resource to the member, family, and interdisciplinary team
Qualifications:
- Active ANP, FNP, or GNP license in the state of New York
- At least one year of Nurse Practitioner experience
- Previous experience with home care and/or geriatrics
