Job description
Conduct initial health assessments for a Medicare/Medicaid plan. This role is primarily home-based and involves in-home work with flexibility for full-time, part-time, and varying availability including nights and weekends. Assessments may take between 15 minutes to an hour.
Job Responsibilities
Order diagnostics, develop and adjust treatment plans, and manage medications. (as applicable)
Educate on disease prevention, medication use, lifestyle changes, and self-care
Complete and submit visit documentation within 24 hours
Qualifications
At least 1 year of experience
Ability to commit to 30 assessments within 1 month
