This healthcare facility is part of a comprehensive health system providing elder care services across multiple locations including Duluth, MN, Virginia, MN, and Superior, WI. The organization focuses on delivering high-quality, patient-centered care with an emphasis on bringing healthcare services directly to where patients reside.

Opportunity:
Advanced Practice Provider - Elder Care

Job Details:
• Full-time (1.0 FTE) NP/PA position providing gerontology-focused care to patients • Utilize advanced health assessment, knowledge, and decision-making skills both independently and in collaboration with physicians • Provide health care to individuals and families with emphasis on health promotion and disease prevention • Work onsite at patient residences, reducing burden on families and patients who would otherwise need to travel to clinics • Must be comfortable working autonomously offsite with collaborating physician partner available by phone • Schedule: 7:00 a.m. to approximately 5:00 p.m. daily • Involves after-hours on-call rotation, including weekends and holidays • Requires high level of customer service skills to establish positive relationships with patients, co-workers, and others • Education required: Master's or Doctorate degree in Nursing or Master's degree in Physician Assistant Studies from an accredited program • Must have current licensure as a Nurse Practitioner or Physician Assistant in Minnesota • Wisconsin licensure required within 3 months of hire • National certification in applicable area and current CPR certification required • Successful completion of credentialing process prior to practice • Ideal candidate will have experience as an RN or NP/PA with elder care background

Compensation:
Competitive hourly rates with weekly pay and malpractice included. May be eligible for additional compensation in the form of bonuses, quality incentives or production-based compensation.

Estimated Annual Salary: $120000 - $140000
