Opportunity: Nurse Practitioner Case Management
*Full Time Permanent
*Schedule: Monday-Friday 8am-5pm and Participate in on call panel
*Provide Comprehensive medical care to PACE Program participants, attends to acute and chronic medical needs and pays attention to preventative and rehabilitative care. Incorporates entire interdisciplinary team as appropriate to provide medical care in a holistic manner.
*Experience in community clinic and/or managed care system and experience with the elderly patient population is a plus
*Comfortable with supervision of Nurses
*Collaborative relationship to physician and/or medical director
Requirements:
*2 years experience minimum
*Experience working with Adult and Geriatric Patients
*Active CA License, BC
*CA DEA
Compensation: Competitive Salary based on level of experience and full comprehensive benefits.
$66-87/HR
