IU Health Physicians is seeking a Nurse Practitioner or Physician Assistant to join our Primary Care team at IU Health Connected Care- Indianapolis!

Connected Care provides comprehensive primary care for adults on Medicare. By providing longer visit times, improved coordination of care with specialists, and more frequent appointments, we strive to improve the experience and outcomes for patients dealing with multiple chronic and/or social health issues. Connected Care providers and staff embrace the outcome-oriented mission of our work and, in parternship with our on-site care management team and on-site pharmacist, we use a team-based care approach to deliver operational and clinical excellence - "Every patient, every visit, every time."

Position Details:

  • Full-time
  • 8a-5p M-F (Wednesday 8a-12p)
  • Days
  • No weekends or call, allowing for work/life balance
  • 100% outpatient
  • Expected patient volume is 64/week, seniors with multiple chronic health conditions
  • Team includes 1 physician and 1 advanced practice provider

Qualifications:

  • ACNP, AGNP, FNP or PA certification required
  • 2-3 years of experience
  • Experience working with complex adult patients who have multiple chronic or social health issues
  • Experience in internal medicine, family medicine with geriatrics, or critical care
