Job Responsibilities
- Provide primary and acute care to nursing home residents.
- Conduct clinical assessments and identify changes in patient conditions early.
- Establish individualized care goals and advance care preferences.
- Coordinate care with families, facility staff, providers, and an RN Care Coordinator.
- Help prevent unnecessary hospitalizations through proactive, on-site patient management.
Qualifications
- Master’s Degree in Nursing (MSN).
- Active Texas Nurse Practitioner license.
- Current Nurse Practitioner certification.
- DEA prescriptive authority.
- Ability to work independently and collaboratively.
