Job Responsibilities

• Provide primary and acute care to nursing home residents.
• Conduct clinical assessments and identify changes in patient conditions early.
• Establish individualized care goals and advance care preferences.
• Coordinate care with families, facility staff, providers, and an RN Care Coordinator.
• Help prevent unnecessary hospitalizations through proactive, on-site patient management.

Qualifications

• Master’s Degree in Nursing (MSN).
• Active Texas Nurse Practitioner license.
• Current Nurse Practitioner certification.
• DEA prescriptive authority.
• Ability to work independently and collaboratively.
