Job description
Job description
Jencare is transforming healthcare for seniors in Philadelphia, Pennsylvania. We provide innovative solutions to complex challenges in healthcare delivery. As a full-risk, value-based primary care leader, we combine a unique physician culture with end-to-end customized technology to deliver world-class coordinated care to our most vulnerable patients—moderate- to low-income seniors with complex chronic diseases.
Job Responsibilities
Plan patient care based on in-depth knowledge of the senior population and relevant protocols. Anticipate and identify physiological and psychological problems commonly encountered, considering each patient's cultural background, level of understanding, personality, and support systems. Serve as a patient advocate throughout the care process.
Manage patient care by performing the following activities:
Write admission, transfer, and discharge orders
Order and interpret appropriate laboratory and diagnostic studies
Order appropriate medications and treatments
Refer patients for specialty consultation when indicated (dermatology, neurology, ophthalmology, endocrinology, surgery, intensive care, infectious disease, hematology, psychiatry, social services, nutrition, and other specialties as needed)
Document care through in-depth progress notes and clinical summaries
Perform invasive procedures independently upon completion of documented competency.
Participate in patient care rounds and conferences. Communicate patient management strategies to members of the care team and collaborate with multidisciplinary team members to ensure care strategies successfully meet patient needs.
Recognize situations requiring immediate physician attention and initiate life-saving procedures when necessary.
Apply advanced communication skills to problem-solve complex situations and improve processes and service delivery to patients.
Qualifications
