Join a mission-driven community health center serving Brooklyn's diverse neighborhoods. This is an excellent opportunity for a Family Nurse Practitioner who is passionate about providing high-quality, patient-centered care while making a meaningful impact in an underserved community.
Responsibilities
- Provide comprehensive primary care for patients across the lifespan.
- Perform wellness visits, preventive screenings, and chronic disease management.
- Diagnose and treat acute and ongoing medical conditions.
- Collaborate with physicians, nurses, behavioral health providers, and care coordinators.
- Educate patients on preventive care, healthy lifestyles, and disease management.
- Maintain accurate EMR documentation.
Qualifications
- Graduate of an accredited Family Nurse Practitioner program.
- Board Certified FNP.
- Active New York State license or eligibility for licensure.
- Primary care or community health experience preferred.
- Strong communication and patient engagement skills.
- Bilingual English/Creole strongly preferred.
