- See patients independently, with physician collaboration as needed
- Build long-term relationships with patients through longitudinal care
- Focus on in-clinic primary care, care coordination and population health; specialists and hospitals handle specialty and acute care
- Care team: physician, MAs, RNs and care managers
- Outpatient only
- 2 years of primary care experience
- Active, non-probationary NP license
- Active DEA
- Medical, dental and vision

About the Opportunity

Requirements
- MSN, DNP or NP Certificate Program graduate- 2 years of primary care experience
- Active, non-probationary NP license
- Active DEA
Preferred
- Knowledge of HCC documentation, ICD-10 coding and Health Risk Assessments (HRAs)Compensation & Benefits
- Salary: $139,150 – $163,350 (wider range based on experience)- Medical, dental and vision
