Requirements

Active Indiana license (as described in the Order details)

BLS (as described in the Order details)

DEA (as described in the Order details)

DOT certification (as described in the Order details)

Minimum experience referenced: 3 years APP experience (as described in the Order details)

Candidates must have at least two years of clinical experience within the past five years to qualify.
