Adult-Gerontology Nurse Practitioner (ARNP) / Physician Assistant – Skilled Nursing & Long-Term Care | Olympia, WA (BB135529)
Job at a glance
Job description
An independent and growing medical group is seeking a compassionate and clinically confident Advanced Registered Nurse Practitioner (ARNP) or Physician Assistant-Certified (PA-C) to provide comprehensive medical care to adult and geriatric patients in skilled nursing facilities and long-term care communities throughout the Puget Sound region.
This is an excellent opportunity to join a patient-centered practice that prioritizes meaningful patient relationships, clinical autonomy, continuity of care, and collaboration with experienced physicians and interdisciplinary healthcare teams.
Unlike traditional high-volume clinical environments, this practice emphasizes thoughtful, relationship-based care. Most providers spend their workday at one assigned facility, allowing them to develop lasting relationships with patients, families, nursing teams, and facility leadership.
Providers have the opportunity to exercise independent clinical judgment while benefiting from physician collaboration, clinical support, and administrative resources.
The ARNP or PA-C will deliver comprehensive primary and post-acute medical care to adults residing in skilled nursing facilities and long-term care communities.
Responsibilities include managing acute and chronic medical conditions, conducting patient admissions and follow-up visits, completing regulatory visits, coordinating discharge care, managing medications, and collaborating with interdisciplinary care teams.
This position is ideal for a provider who enjoys caring for medically complex adult and geriatric patients and values continuity, clinical independence, and meaningful patient interactions.
Job Responsibilities

Develop, implement, and update individualized treatment plans based on patient needs and clinical findings.
Perform procedures when clinically appropriate and within the provider's scope of practice, training, privileges, and facility capabilities. Additional training may be provided when needed.
Care Coordination & Collaboration
Communicate effectively with nursing staff, patients' families, specialists, pharmacies, and other healthcare professionals.
Coordinate care with facility leadership, attending physicians, and other treating providers.

Utilize available scribe services and advanced AI documentation tools to support efficient clinical workflows.
