At SSM Health, we are committed to delivering exceptional healthcare services. Our outpatient Family Medicine Clinic in North Fond du Lac is seeking an Advanced Practitioner (PA or FNP) to join our supportive outpatient Family Medicine practice.

Minimum Qualifications
- Graduate of nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) Program
- NPs should be APRN
- FNP is required
- Ability to obtain PA/NP licensure in Wisconsin
