Summary: We are seeking a Nurse Practitioner or Physician Assistant to provide patient-centered medical care to residents in skilled nursing and long-term care facilities in Sarasota, FL. This is an excellent opportunity for both experienced providers and motivated new graduates interested in post-acute and long-term care.

Responsibilities:

• Conduct patient assessments, evaluations, and follow-up visits.

• Develop and implement individualized treatment plans.

• Monitor patients' health conditions and adjust treatment plans as needed.

• Collaborate with physicians, nursing staff, therapists, and other healthcare professionals.

• Maintain accurate and timely documentation in electronic health record (EHR) systems.

Qualifications:

• Active Nurse Practitioner or Physician Assistant license in Florida.

• Prior skilled nursing facility experience is preferred, not required.

• Strong clinical assessment, communication, and decision-making skills.

• Ability to work independently and collaboratively with a multidisciplinary team.

• Excellent organizational skills and attention to detail.
