About the Role
As a Nurse Practitioner or Physician Assistant, you'll provide comprehensive primary care services to residents in long-term care and post-acute care settings.
This is an excellent opportunity for providers who enjoy caring for medically complex geriatric patients while practicing in a supportive, team-oriented environment.
You'll care for patients with a variety of chronic and acute medical conditions, including individuals with tracheostomies and ventilators. These patients are already medically managed, and while you'll provide ongoing primary care and medical oversight, you will not be responsible for ventilator or tracheostomy management. Specialized clinical teams provide support for those aspects of care.
Responsibilities
- Perform comprehensive patient assessments and routine follow-up visits.
- Diagnose and manage acute and chronic medical conditions.
- Develop individualized treatment plans.
- Collaborate with physicians, nursing staff, therapists, and interdisciplinary care teams.
- Maintain timely and accurate clinical documentation.
- Promote continuity of care while building strong relationships with patients and facility staff.
Qualifications
- Licensed Nurse Practitioner or Physician Assistant
- Eligible for state licensure
- Passion for caring for geriatric patients
- Strong communication and clinical decision-making skills
- Ability to work collaboratively within an interdisciplinary team
Previous geriatric or long-term care experience is welcomed but not required. New graduates with a passion for caring for older adults are encouraged to apply.
