Position Overview
Provide in-home health assessments, including history and physical evaluations, medication review, vital signs, and required screenings.
Key Responsibilities
Conduct in-home health and wellness assessments
Complete history and physical evaluations and review medications
Document visit information using a provided device and required system
Required Qualifications
MD, NP, or PA
Active, unrestricted Georgia license
NPI required
Must have at least one weekday available (weekend-only availability is not accepted)
Candidates must have at least two years of clinical experience within the past five years to qualify.
