Duties & Responsibilities

• Provide outpatient primary care services to adult and geriatric patients.
  
• Perform preventive care services and chronic disease management.
  
• Provide post-acute care services in SNF and assisted living settings as needed.
  
• Manage patient care across clinic and post-acute settings.
  
• Participate in call coverage (beeper/phone) with a 1:10 ratio.
